Nursing Care
A Registered Nurse (RN) coordinates the plan of care for every patient under supervision of a qualified Hospice Physician (MD).
RN Case Managers provide direct patient care and manage symptoms and medication. The nurse is the link between the patient, his or her family, and the physician. Nursing staff is available 24 hours a day, 7 days a week.
Medical social services
A licensed medical social worker provides advice and support to the patient and his or her family. The social worker assists other care team members in understanding the family dynamics and acts as an advocate for the patient and family. They may assist with funeral planning, long-term care planning, advance directives, and community resources.
Trained volunteers and additional support services
Volunteers provide compassionate and practical support for the patient and their families. Volunteers are thoroughly screened and trained to assist in hospice care.
HOME CARE AIDE SERVICES
Certified Nursing Assistants provide personal care for the patient, such as bathing, grooming and dressing.
Additional SERVICES
Physical & Occupational therapists are also available to care for the patient as needed.
Physician services
The patient's physician directs the plan of care and works closely with the hospice team. The hospice medical director is available to the attending physician, the patient, and the hospice care team.
Bereavement counseling
Bereavement Counseling and support are available at the request of the family members and is a covered part of the hospice program benefit.
Spiritual support and counseling
Chaplains provide spiritual support and guidance when needed, regardless of faith preference. Hospice also utilizes the services of local religious communities and congregations to provide a broader scope of spiritual care.
Medicaid room and board
We accept Medicaid for room and board services provided in nursing homes and long-term care facilities.
